All UToledo students are required to 有健康保险. 体育博彩 has partnered with Anthem 学生 Advantage to provide students with medical 和 prescription drug coverage for the 2023-24 academic 一年.
UToledo automatically 招收s the following students in the student health insurance plan 和 assesses a fee to your student account:
- All domestic undergraduate students registered for 6 or more credit hours per term
- All domestic graduate students registered for 6 or more credit hours per term
- All international students registered for 1 or more credit hours per term
- All students who participate in patient care 和 are required by their program to 有健康保险
你可能 放弃 the health insurance coverage if you have coverage under another qualifying plan as long as you submit your 放弃r by the deadline. Qualifying plans must be filed 和 在美国获得批准.S. 并符合ACA标准.
If you don’t 放弃 the insurance by the deadline, you will be 招收ed in UToledo’s student medical 和 prescription drug insurance plan.
This student health insurance plan 招收ment model is a best practice in Ohio. It has allowed UToledo to negotiate lower premiums with Anthem 学生 Advantage.
Degree-seeking domestic students taking less than 6 credit hours per term 和 eligible dependents of all 招收ed students may voluntarily 招收 in the student health insurance plan directly with Anthem 学生 Advantage.

The student health plan administered by Anthem 学生 Advantage provides access to nationwide providers 和 in the area surrounding UToledo, including the University of Toledo Medical Center, University Health Center 和 the 学生 Health 和 Wellness 健康科学校园中心.

Certain populations are eligible to voluntarily 招收 in the student health insurance 计划:
- Degree-seeking students taking 1-5 credit hours
- Dependents of UToledo students 招收ed in the student health insurance plan
- UToledo students 和 their eligible dependents who want 牙科 和/or 愿景 coverage
Note: Only students are eligible to use the University Health Center 和 the 学生 Health 和 Wellness Center on the Health Science Campus. 配偶及子女可 use University of Toledo Physicians, University of Toledo Medical Center facilities 的其他区域提供程序 国歌网络.

- In the Toolkit (top left of your screen), select the Health Insurance Plan – Enroll or Waive link under the My Registration Steps section.

To 招收 for 牙科 和/or 愿景 insurance 和/or medical 和 prescription drug coverage for your dependents (spouse/children):
- 登录 myUT门户 使用您的UTAD和密码.
- In the Toolkit (top left of your screen), select the Health Insurance Plan – Enroll or Waive link under the My Registration Steps section.
- Select Add Dependent/配偶 or 愿景/牙科.
How to Waive 学生 Insurance Plan

- Under My Registration Steps, select Health Insurance Enroll, Change, or Waive.
- 选择期限.
- Proceed with 招收ment or 放弃r option.
The student health insurance premium will be removed from your student account as
long as the 放弃r is received prior to the semester’s deadline.

- Summer 2024 (for newly 招收ed UToledo students only): 2024年7月1日
NOTE: Once your 放弃r has been accepted, it will remain on file for the 2023-24 academic 一年. A new 放弃r is needed each academic 一年.
